What Is Sugar?

Is Sugar Essential for the Body?
One common claim is that sugar is toxic. This isn't supported by the evidence. Sugar is essential in the human body.
Your blood is a solution of glucose and many other substances, and your body works hard to maintain blood sugar levels within a very tight range. Some organs, including the brain, need a continuous supply of glucose to function. Even if you eat no glucose or carbohydrates at all, your body can still produce glucose from components of protein and fat in a process called gluconeogenesis. If glucose were truly toxic, your body would not need to manufacture it to survive.
What Happens When We Eat Sugar?
When you eat carbohydrates, whether that's fruit, honey, chocolate, bread, or a potato, your body digests them quickly, absorbs the glucose, and your blood glucose rises. This is completely normal. If you're healthy, your body brings that glucose back down to normal levels quickly. Spikes in blood sugar after eating are a normal part of how the body works.
Equally, when blood glucose drops too low between meals or during sleep, your body can raise it back to a healthy level. This is part of a process called homeostasis, where the body keeps all its systems in balance.
Does Sugar Make You Gain Fat?
A common misconception is that eating sugar causes immediate fat gain. This idea is closely tied to insulin, and to a misunderstanding of how insulin works.
When you eat carbohydrates, they're digested to sugar, which enters the bloodstream and triggers insulin production. Insulin's job is to lower blood glucose. It does this by:
- Increasing the burning of sugar for energy
- Reducing the burning of fat for energy
- Causing the body to store carbohydrate either as glycogen in the muscles or by converting it to fat in fat cells
This is why many people are wary of sugar and insulin. However, insulin only has these effects when its levels are elevated, which is for a short period after eating. After a couple of hours, insulin drops and the body returns to burning fat, including stored body fat.
We also have good evidence that people can lose weight even when their insulin levels are elevated. There was a diet in the 1940s and 50s called the rice diet, in which more than 90% of calories came from rice and fruit, both of which are digested down to sugars and spike insulin. People on the diet lost weight regardless.1,2,3,4 The reason is straightforward: they were eating fewer overall calories. At the end of the day, it is total calories that drive weight loss.
Is Fructose the Real Problem?
Some argue that fructose specifically is to blame. Fructose is metabolised differently from glucose, and one distinction is that it may be more readily converted to fat in the liver, a process called de novo lipogenesis. However, this does not cause weight gain unless a person is eating more calories than they need. We have clear evidence that people on high-fructose diets do not gain weight as long as they are not eating excess calories.5
What About High Fructose Corn Syrup?
High fructose corn syrup (HFCS) is produced industrially by fermenting corn. It's cheap, which is why it's become a common ingredient in processed foods. The concern that it's significantly worse than regular sugar is largely unfounded.
Table sugar is already 50% fructose. High fructose corn syrup typically has a maximum fructose content of 55%. That minor 5% difference is not large enough to have a meaningful impact on human health.
So What Is the Real Problem With Sugar?
The real issue is not sugar itself, but the way it's used in the modern food environment.
Since the 1970s, food availability has skyrocketed, and a larger portion of it is processed or ultra-processed. Many ultra-processed snack foods have four characteristics that make overeating easy6:

But there is no unusual biochemistry or special mechanism that makes sugar uniquely fattening. We simply tend to eat too much of it.

If you have a healthy diet and are not eating too many calories overall, having some sugar in your diet is absolutely fine. It will not automatically cause weight gain.
